Our office hours are Monday through Friday 8:00 AM to 5:00 PM.

If you are unable to keep an appointment, please give 24 hours notice. This enables us to schedule another patient and you'll avoid charges for a missed appointment.

Insurance:

We participate with most private insurance carriers (click here for a list), Medicare and AHCCCS. You must bring your card with you. You are responsible for any amount not covered by your plan. Payment is required at the time of service. If you need to arrange a payment plan, we will assist you.
